Partners in the Community

At Willowbrook Hospice, our care is strengthened by the many partnerships we have in all the communities we serve. This includes hospitals, physicians, assisted living communities, and long-term care centers.  

Our partnerships ensure that each individual seeking hospice care from Willowbrook will benefit from a smooth transition from facility to home or home to facility as needs arise. We work diligently with our medical directors and any chosen attending physicians to maintain quality care.   

We are also active as members of the National Hospice and Palliative Care Organization and the We Honor Veterans Program.
